What is ICSI?

Intracytoplasmic Sperm Injection (ICSI) is a specialized technique within In Vitro Fertilization (IVF) that is used to overcome severe male fertility problems. This procedure consists of the direct injection of a single sperm into the egg, facilitating fertilization when other techniques have failed.

What is the process like?

1. Ovarian Stimulation

The woman receives medications to stimulate the ovaries to produce multiple mature eggs.

2. Egg and Sperm Recovery

The eggs are retrieved from the ovaries using a minimally invasive technique. The sperm is prepared in the laboratory or extracted directly from the testicles if necessary.

3. Sperm Injection

An embryologist selects the most viable sperm and injects it directly into the egg with a microscopic needle.

4. Embryo Culture

After fertilization, the embryos are cultured for a few days in the laboratory to evaluate their development.

5. Embryo Transfer

Viable embryos are transferred to the woman's uterus, where they are expected to implant and lead to a pregnancy.

6. Pregnancy Test

Two weeks after the transfer, a pregnancy test is performed to determine if the treatment was successful.

When is it recommended?

ICSI is especially useful in cases where:

  • Low sperm count (oligospermia)
  • Sperm motility is reduced (asthenospermia)
  • There are problems in the morphology of the sperm
  • There is an obstruction that prevents sperm from reaching the semen
  • Vasectomies have been performed and fertility recovery is desired
  • Sperm has been obtained by testicular biopsy

The main difference is the fertilization method. In conventional IVF, eggs and sperm are placed together in a laboratory so that fertilization occurs naturally. In ICSI, a single sperm is injected directly into the egg, increasing the chances of success in cases of severe male infertility.

ICSI is generally safe, but as with any medical procedure, there are some risks. These include the risk of damage to the egg during the injection.

Success rates of ICSI are generally high, especially compared to conventional IVF in cases of male infertility. However, success depends on several factors, such as the quality of the egg and sperm, and the age of the woman. In general, pregnancy rates are between 60% and 80% per cycle in women under 35 years of age.

In cases where viable sperm are not found in the semen, a testicular biopsy can be used to try to recover sperm directly from the testicle. If this is also not possible, using donor sperm can be considered as an alternative.
